This session is ideal for: Family caregivers, direct support professionals, healthcare providers, case managers, aging network professionals, and others supporting older adults with IDD.

Objectives
  • Differentiate the major Kansas HCBS waivers and identify eligibility requirements and available supports through a 4Ms lens (what matters, medication, mentation, and mobility).
  • Describe the process for accessing HCBS services, including the roles of CDDOs, ADRCs, and Managed Care Organizations.
  • Evaluate person-centered strategies for navigating waiver transitions and maintaining continuity of supports as the needs of older adults with IDD change.
