  • 2. Explain how different thinking modes, i.e., GEM (S-R-K) classification, aid in understanding human errors.
  • 3. Differentiate how system flaws and human error contribute as causes of adverse events.
  • 4. List three error prevention tools that physicians can use to prevent errors leading to patient injury.
  • 5. Explain how important it is for Medical Staff to encourage the Hospital Staff to use error prevention tools consistently.
